Position Summary: Provides overall leadership, direction, and strategic planning for Case Management services to promote and ensure the advancement of coordinated care across the care continuum. In coordination with leadership, ensures that case management and care coordination services effectively and efficiently meet the needs of all internal and external customers and are compliant with regulatory groups.
Experience
Required: Three (3) to five (5) years progressive leadership experience as a Registered Nurse; and three (3) to five (5) years in case management. Preferred: N/A Education
Required: Bachelors’ degree in nursing required Preferred: Discipline-specific master’s degree or MBA or MHA License and Credentials
Required: NJ State RN License; Certification in Case Management within one year Preferred: BLS Healthcare Provider Certification Skills
The starting salary range for this position is $110,000 - $145,000
When determining a team members base rate, several factors may be considered as applicable (e.g., years of recent relevant experience, education, credentials, and internal equity).
